Educational Questions

Educational Questions

Do you offer provider discounts?

Yes, providers can request discount codes or affiliate links to reduce costs for patients and earn commissions on referrals. Mira also provides a public discount code for social media or webinars.

My patient is taking oral progesterone, and I’ve observed occasional drops in her PdG levels. What could be the reason for this variability?

Progesterone levels can fluctuate due to factors like timing of collection, hydration, or recent activity. Focus on trends over time rather than isolated measurements.

How can I better understand Mira’s technology to educate my patients?

Contact Mira for personalized training or access their resource hub, which includes videos and case studies tailored for healthcare providers.

Where can I find a provider training course about monitoring women’s hormones?
What medications, supplements, interventions are contraindicated with Mira?

To ensure accurate results when using Mira for testing, it’s important to understand how the following medications and supplements may affect the data:

1. Methylene Blue
◦ Do not take Methylene Blue while using Mira for testing.
◦ Discontinue Methylene Blue 5-7 days prior to testing to avoid interference.

2. Biotin

◦ High doses of Biotin (5,000 mcg or more per day) can disrupt urine hormone tests, particularly affecting hCG levels in urine and TSH, T4, and T3 levels in blood.

◦ Biotin has a half-life of 2 hours, but its metabolites can remain in the body for up to 24 hours. Discontinue high-dose Biotin supplements at least 48 hours before testing.

3. Vitamin C
◦ Doses greater than 1,000 mg per day of Vitamin C may impact hormone metabolite results in urine.

4. Pregnenolone
◦ Pregnenolone supplementation can affect Mira results by increasing progesterone metabolites due to the first-pass effect, although it does not directly raise progesterone levels.

◦ To monitor endogenous hormones, discontinue oral or sublingual pregnenolone for 3 days before testing.

5. DHEA

◦ DHEA supplementation can also affect Mira results.

◦ To monitor endogenous hormones, discontinue DHEA at least 1 day before testing.

6. Other Medications, Supplements, and Interventions

◦ Other medications and supplements may also affect Mira data.

Please refer to our detailed protocols for specific guidelines.

What other factors can affect Mira data?

Fluid intake and hydration status can influence data, as well as medications like Clomid. Kidney disorders may alter urine composition and affect accuracy.

Is Mira creatinine corrected?

No, creatinine correction is not necessary if patients follow fluid intake guidelines, as it does not significantly impact accuracy.

What is the reliability and validity of Mira?

Mira is reliable and valid when used correctly. It tracks hormone trends accurately and align well with serum hormone levels, as demonstrated by clinical research. Internal checks and calibrations ensure consistent performance.

Additionally, Mira’s coefficient of variation (CV) falls within industry standards, and internal studies comparing Mira to the Abbott Architect i2000SR have shown a strong linear correlation.

Does Mira track testosterone?

We currently do not track testosterone.

Can Mira be used while on birth control?

Hormonal birth control typically suppresses ovulation, but some women may still ovulate and have menstrual cycles. Combined oral contraceptives typically suppress ovulation, which can result in non-ovulatory, non-menstrual bleeding cycles, breakthrough bleeding, or even amenorrhea. In contrast, progestin-only pills (POPs) may allow for the continuation of regular cycles.Mira can be useful for women experiencing symptoms they attribute to hormonal fluctuations or deprivation while using contraceptive methods that suppress ovulation. For example, if symptoms such as vaginal dryness, pain during sex, low mood, or low desire are present, Mira can help confirm low hormone levels and the absence of fluctuating LH/FSH, supporting the possibility of hormonal deprivation. If symptoms appear cyclical but Mira shows no hormonal fluctuations (as expected with ovulatory suppression), clinicians can help differentiate these symptoms from hormonal causes. Mira can also help identify background ovarian activity, whether it is regular, irregular, or resulting in successful ovulation. Some healthcare providers recommend that patients begin using the Mira monitor to track their hormone patterns before transitioning off birth control. This approach helps patients establish a testing routine and gain a clearer understanding of their hormonal changes both before and after discontinuing birth control. It provides valuable insights into their fertility and overall reproductive health throughout the transition period. See additional information here.

Is this applicable to post-menopausal women at all?

Beyond two years post-menopause, hormone levels are generally low and stable, making tracking with Mira likely unnecessary. However, using Mira to monitor hormone replacement therapy (HRT) during menopause is still being investigated.

Is there any data for perimenopausal women?

Yes, several pilots and studies are currently underway related to perimenopause. Our perimenopause use case resources are here

Does it track xenoestrogens? Does this only detect natural estrogen or if there is a woman with high xenoestrogens from outside sources, would it show estrogen high?

Mira does not track xenoestrogens directly. However, if xenoestrogens lead to an increase in serum estradiol (E2) levels or contribute to estrogen dominance, Mira’s E3G levels will reflect this increase.

What does it mean if PdG is elevated in the follicular phase?

It is important to note that PdG is highly sensitive to improper testing techniques, so we strongly recommend that clients carefully follow the provided instructions to ensure accurate results. Anecdotal observations suggest that abnormal PdG levels during the follicular phase may occur in individuals with hormone imbalances, such as those with PCOS, although further research is ongoing. Additionally, a study has reported stress-induced increases in both progesterone and cortisol in naturally cycling women (reference).  Currently, we do not consider elevated PdG levels in the follicular phase to require specific treatment.

Is there a threshold of PdG that is confirmatory for ovulation?

To confirm an ovulatory event using Mira PdG, you can look for one of these two patterns:

Option 1- Three-Day Rise: Three consecutive days of rising PdG levels, with each day at least 1.25 times higher than the previous day after an LH surge.

Option 2 - Sustained Elevated PdG: Multiple PdG levels consistently higher than baseline following an LH surge.

For additional detail please refer to our interpretation guide.

How do urinary metabolites correlate with serum levels?

While research involving Mira has shown a strong correlation between serum hormone levels and urinary metabolites, it's crucial to understand that they are not directly equivalent (see research here). Urinary metabolites and serum hormones are measured in different units and reflect a correlation rather than a direct equivalence. Urinary metabolites, which are a result of the metabolism of serum hormones, offer a delayed reflection of these hormones. While urinary metabolite levels may fluctuate, they generally align with the overall pattern of serum hormone levels. Therefore, instead of comparing specific data points, it's more effective to focus on trends over time. Many healthcare providers utilize a combined approach, employing continuous monitoring with devices like Mira to track trends and periodic serum tests to obtain precise snapshots of hormone levels at specific points in time. In conclusion, while Mira can be a valuable tool for monitoring hormone trends, it's important to remember that it's not a substitute for traditional serum hormone tests.

How does Mira compare to other tracking devices such as Inito or Proov?

Mira uses fluorescent lateral flow immunoassay technology, which is more accurate and sensitive than the nano-gold technology used by other trackers (e.g., Proov, Inito). Fluorescent analyzers, like Mira, provide quantitative results preferred by providers. Read more here.

How does Mira compare to Dutch?

Mira offers several advantages over traditional dried urine testing, such as the Dutch test, by utilizing laboratory-grade immunofluorescence technology, providing immediate results, and enabling remote monitoring. These features make Mira a highly efficient and convenient option for tracking hormone levels and trends in real time. In contrast, Dutch requires a dried urine sample to be collected, mailed, and analyzed at the Dutch laboratory, which typically takes up to three weeks to return results. Additionally, Mira and Dutch use different sets of urine metabolites, which means their results cannot be directly compared.

While Mira can replace Dutch cycle mapping, it can also complement Dutch testing, which provides insights into adrenal function, organic acids, and metabolite pathways. For instance, Mira can help accurately time interventions and assessments, such as conducting a Dutch Complete test seven days after ovulation. This approach is more reliable than using a generic cycle day 21 result, as ovulation timing varies significantly between women. Testing seven days post-ovulation offers a more precise reflection of hormonal activity.
While Dutch offers a detailed snapshot of hormone levels, Mira's real-time monitoring and trend analysis provide continuous insights, making it particularly beneficial for ongoing interventions and personalized healthcare strategies.
